Our Covid Policy for Visits

Eldementals, LLC is constantly monitoring current research and guidelines from the CDC, WHO, and VDH.  Our primary goal is to continue to provide the highest level of care management possible, while minimizing risk of exposure to our clients.  

All Eldementals, LLC Care Management Staff at present have received the Moderna Vaccine. 

We ask that all patients and family members read below and comply with the following for onsite services and visits: 


  • We encourage Virtual Care Management access to our services if possible at this time. We use the doxy platform for this.  Essential and emergent home and care community visits will still be offered and supported.  

  • Wear a cloth or disposable surgical mask for all visits with Eldementals staff members.

  • Maintain minimum social distancing of 6 feet during visits.  Minimize the number of individuals attending onsite meetings - only those truly essential are to be onsite at visit.  

  • Only the patient(s) and 1 essential family member or other party directly associated with care in addition to the care management staff may attend onsite visits. No more than 3 people total may attend a visit.  

  • Ensure that visit takes place in a well ventilated area - outdoor spaces are ideal if appropriate and possible. If needing to have more than 3 individuals involved in a meeting this may be best completed via Eldementals tele health - doxy or by telephone as we will provide a tele conferencing dial in for use.  

  • Special considerations will be made for and additional instructions will be given to those who are critically ill with limitations and on hospice, end of life care.  

  • Our staff will wear essential protective equipment  as appropriate based on the care situation for safety during all visits.

  • We will check temperatures upon arrival and ask basic health questions.

  • Prior to your visit: Complete online the brief coronavirus self checker below prior to each visit: https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/coronavirus-self-checker.html

  • If unable to do the above online please contact us promptly prior to visiting to reschedule if the following is noted:  Any fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea in past 24 hours, if you have a covid test result pending, have been within 6 feet of a person with a lab-confirmed case of COVID-19 for at least 5 minutes, or had direct contact with their mucus or saliva in the past 14 days or have tested COVID positive in the past 14 days.  

  • We are unable to do an onsite visit for those with active covid, covid symptoms, a pending test, or who have traveled out of state until socially isolated for a 14 day period.  

  • For safety, routine care management and crisis management services will still be provided services via tele-health platform and by telephone teleconferencing.
