Care Management a Benefit for Children With Complex Care Needs


It is well known that Care Management can significantly impact overall quality, outcomes, and cost for those with chronic or complex conditions at any age. Studies indicate however that children with the following conditions especially benefit from care management services:

Care Managers guide clients and families in navigating the complexities of the continuum of care and in providing and linking them to available resources for long-term care planning and support. They serve as an advocate, liaison, and expert in their field.

What do Care Managers do?

Care Managers work in collaboration with Physicians, Hospitals, Discharge Planners, Elder Care Law Attorneys, Individuals and Families impacted by acute, chronic, or degenerative conditions, Banks and Trusts, Guardians, conservators, and Powers of Attorney, SNF, AL, IL, Rehab and LTC communities, Home Health agencies, PT, OT, Speech, Nutrition, Hospice and Palliative Care Organizations, Adult Protective Services, Adult Day care Centers, Workmen, Builders, Social Workers…

Care management is a key component in ensuring quality outcomes for clients with complex disease processes!
