New Extended Care Program
From time to time, I like to highlight providers who are going above and beyond in terms of service, innovation, and quality.
Allegiance Home care is located in the Northern Virginia area and recently started a new and innovative program to assist individuals to heal and age in place - The Extended Care Program.
So how does The Extended Care Program work?
The Extended Care Program is a customized program for individuals who are transitioning home from a hospital, rehab, or other acute/sub-acute setting.
The program provides a variety of essential services to cover all care related areas. It combines both hourly or live-in care provided by a qualified private duty care professional and house calls by physicians and nurse practitioners.
Some of the other services under this program include:
Unpacking, laundry, grocery list, etc.
Monitoring of vital signs
Light housekeeping and meal preparation
Physician or NP visits within 48 hours of discharge
Full medication reconciliation
A care plan for ongoing care
Assistance with medication administration
Their unique, all inclusive model combines home care with physician house calls, thus ensuring that a patient's transition home is stress free, safe, and cost effective. This one stop shopping approach truly takes the stress out of the equation and provides peace of mind. This inclusive package provides discounted pricing which allows individuals to also plan for and budget costs along the way for the critical services needed while healing.
If you are interested in learning more about services such as this, please reach out via our contacts tab. An RN care manager can assist in ensuring that care is well coordinated and managed long term and not just during the initial return to home. RN Care Managers provide 24 hour, 7 day a week access to care.
