So how does The Extended Care Program work?
The Extended Care Program is a customized program for individuals who are transitioning home from a hospital, rehab, or other acute/sub-acute setting.
The program provides a variety of essential services to cover all care related areas. It combines both hourly or live-in care provided by a qualified private duty care professional and house calls by physicians and nurse practitioners.
Some of the other services under this program include:
Transportation home
Unpacking, laundry, grocery list, etc.
Monitoring of vital signs
Light housekeeping and meal preparation
Physician or NP visits within 48 hours of discharge
Full medication reconciliation
A care plan for ongoing care
Assistance with medication administration
